Absence Report and Planned Time Off Request

Every paycheck adds time off to your bank. It is yours, and it has a limit. Every hour you take comes out of it. You manage it. Check your balance in Gusto.

Every absence and late arrival is recorded. Too many may result in discipline.

Time off for a covered purpose, like being sick or a medical appointment, comes out of your bank and does not count against you. Once your bank is empty, it counts like any other absence.

Is this absence for a covered purpose?

Any medical, dental, vision or therapy appointment counts, yours or your family's, and so does preventative care. Being sick, or caring for a child, spouse or parent who is, counts.
What counts as a covered purpose?
  1. Caring for your own physical or mental illness, injury, or medical condition, including preventive care.
  2. Attending your own routine medical or dental appointment.
  3. Caring for your child, spouse, parent, or spouse's parent who has a physical or mental illness, injury, or medical condition, including preventive care.
  4. Taking your child, spouse, parent, or spouse's parent to a routine medical or dental appointment.
  5. Addressing the psychological, physical, or legal effects of domestic violence affecting you or your dependent child.
  6. Addressing your own physical and mental health needs, or those of your spouse, arising from pregnancy loss or a failed assisted reproduction, adoption, or surrogacy.
  7. Travel to and from an appointment, a pharmacy, or another location related to any purpose above.
You never have to tell us what is wrong, and we will never ask.
Not a covered purpose: a school or daycare closure, a transportation problem, a home repair, a non-medical appointment. You can still report these. Answer No.
